Getting Started

What can you expect when you visit us for the first time?

You will be asked to fill out a patient information form and medical history. A copy of this form and

our patient brochure is mailed to you in advance if there is time before your appointment.

You will be asked to present your insurance card and a picture ID. These cards will be scanned

into our medical management computer system and you will only have to present them at your first visit

and upon change in insurance or address.

You will be asked to present your referral if required from your primary care physician.

You will be asked for co-payments prior to seeing the doctor.

What information should you bring?

Medical records including test results and films.

Medication list or medication bottles if possible.

Contact information for your primary care practitioner including phone number, fax number and mailing address.

List of questions for the doctor.

A companion or interpreter if necessary.

 Following Up

Appointments will be scheduled as needed to evaluate your progress, health management,

and to monitor any new cardiac conditions. What can you expect on on a follow up visit?

  • A report of findings and explanation of results of testing and management options.
  • A review of all  medications.
  • A review of any new cardiac problems.
  • A follow-up scheduled appointment at medically appropriate intervals.
